Do not take alendronate if you have certain disorders of the esophagus, are unable to stand or sit upright for at least 30 minutes, are allergic to any of the ingredients, have severe kidney disease, are pregnant or breast feeding, are under 18 years of age, or if your doctor has told you that you have low blood calcium. Discuss with your doctor or pharmacist any and all medical problems you may have or had prior to taking alendronate.

Alendronate 70mg is taken once weekly the same day every week. After getting up for the day and before taking your first food, beverage or other medication, take alendronate with a glass of plain water only. Do not take with mineral water, coffee or tea, or juice. Do not chew or suck on tablets of alendronate. After taking alendronate, do not lie down – stay fully upright (sitting, standing or walking) for at least 30 minutes and do not lie down until your first food of the day. Do not take alendronate at bedtime or before getting up for the day. Alendronate is taken over the long-term and should not be stopped without consulting your doctor.

Most patients do not have side effects from taking alendronate. The most common side effects include abdominal pain, dyspepsia (indigestion), acid regurgitation, nausea, constipation, diarrhea, flatulence, and gastritis. Bone, muscle or joint pain is also a common side effect of alendronate. In rare cases, the following may occur: osteonecrosis of the jaw, allergic reactions, symptoms of low blood calcium and sever irritation or ulceration of the esophagus. If you develop difficulty or pain upon swallowing, chest pain, or new or worsening heartburn, stop taking alendronate and call your doctor. For a complete list of possible side effects, contact your doctor or pharmacist.

Osteonecrosis of the jaw is a rare condition that involves the loss, or breakdown of the jaw bone. This has been reported in patients with cancer receiving treatment with this type of medication. Report to your doctor any symptoms of pain, swelling, gum infections, loosening of the teeth, poor healing of the gums after dental work and numbness or feeling of heaviness in the jaw. Consult your doctor if you need major invasive dental treatment while you are taking alendronate. Let your dentist know if you are taking alendronate.

Calcium supplements, antacids, and other oral medications must be taken at a different time than alendronate as they will interfere with its absorption. Patients must wait at least one-half hour after taking alendronate before taking any medication or supplements.

In addition to taking alendronate, your physician may recommend the following lifestyle changes: stop smoking as smoking appears to increase the rate at which you lose bone; exercise as bones require exercise to stay strong and healthy; eat a balanced diet to include calcium and foods that support bone strength. If you take too much alendronate, drink a full glass of milk and contact your doctor immediately – do not lie down.

If you miss a dose, just take one tablet on the morning after you remember – do not take two tablets on the same day.
